Why does my 4 month puppy pee inside after being outside?

She drinks a lot of water, but why doesn't she just release it all outside? I take her out every hour since it seems she has no control over her bladder whatsoever even at 4 months, but then she comes inside and just pees a huge puddle on the carpet...How long will it take for her to stand at the door when she needs to go? Also, she never poops "15 minutes after eating", so I never know when to take her out to poop and she just ends up sh*tting on the floor randomly. -___-
